INSURANCE CASES AND PRIVATE PAY The Agency may bill insurance companies following verification of coverage including: Services covered Coverage limitations Co-payments Maximum lifetime benefits Required medical documentation Billing process Assignment of benefits Patient responsibilities Agency responsibilities Physician responsibilities Contact person If the insurance company approves an assignment of benefits, the authorization from the patient to assign benefits shall be obtained. If the insurance company will not accept assignment, this will be noted on the service agreement and the patient or responsible party will be informed of their liability, if any, for payment. Private pay patients will be notified of the Agency's charges in writing at the time of referral. Private pay accounts shall be billed and paid weekly unless other arrangements have been made.
