MDT Home Health Care Agency, Inc. statewide toll-free telephone number for the central abuse hotline will be provided to clients in a manner that is clearly legible and will include the words: "To report abuse, neglect, or exploitation, please call tollfree(1-800-962-2873)." In any case and every case where an employee of this Agency, in the course of her normal duty, is involved in any act that constitutes abuse, neglect or exploitation, we urge our patients to call our office immediately. They are informed that should also call the abuse registry Hot line at 1-800-96 ABUSE. TO REPORT ABUSE, NEGLECT OR EXPLOITATION, CALL TOLL FREE 1-800-96-ABUSE (800-962-2873) A label with the following information is included in all sign up packages: At Admission date the patient will be informed of the availability of telephone hot line for questions, fraud reporting, information on Advance Directives or complaint from: Accreditation body (if applicable), CMS (Medicare or Medicaid beneficiary), State Regulatory Agency, and our Agency.
