QUALIFICATIONS: Must be a licensed Physician, or Registered Nurse, or holds an undergraduate degree, and has at least 1 year of supervisory or administrative experience in home health care or related health programs. Be knowledgeable of the federal conditions of participation (CoPs), statute and administrative rules of the State, and ensure compliance with them by the Agency. Must show exceptional leadership ability and good professional judgement. Maintains administrative and/or supervisory responsibilities when care is provided under contract. If our Agency changes the administrator or alternate administrator, we shall notify the State, Home Care unit office prior to or on the date of the change. Notification shall consist of submission of the person’s name, professional resume, professional license, if applicable, and a copy of the Affidavit of Compliance with Criminal Background Check .
