MDT Home Health Care Agency, Inc. continuing care, Ensuring that referral forms, plans of care, service reports, and all other clinical records are properly and promptly written and transmitted to responsible physicians and to contracting agencies, including third-party payers, Supervising, guiding, and directing Directors in the areas of policy and operation, caseload management, and personnel management, Ensuring that policies and procedures guiding clinical service provision are developed and implemented, Advising the President regarding staffing needs and requirements and resources for services, Providing leadership in development and implementation of a performance improvement (QAPI) program, Providing direction in developing, implementing, and evaluating orientation and staff development programs, Supervising preparation of payroll data, Supervising all billing activities. ·Director of Nursing, Clinical Manager The Director of Nursing, Clinical Manager is responsible for provision of patient care services and day-to-day operations of the Clinical division he/she is responsible for the following functions, either directly or through delegation to qualified staff: Participating in all activities relevant to the daily operations of the Clinical division Coordinating and integrating the services provided to patients according to their attending physician’s orders; ensuring that services are available as needed for agency patients and as presented to the public Oversees the development of discharge plans for home health patients in cooperation with attending physicians, nurses, and therapists, and when necessary, making suitable arrangements for patient’s continuing care Ensuring that referral forms, plans of care, service reports, and all other clinical records are properly and promptly written and transmitted to responsible physicians and to contracting agencies including third-party payers Supervising, guiding, and directing clinical supervisors in the areas of policy and operation, caseload management, and personnel management Developing and implementing policies and procedures guiding clinical service provision are developed and implemented Advising the President regarding staffing needs and requirements and resources for services Providing leadership in development and implementation of a performance improvement (QAPI) program Oversees the developing, implementing, and evaluating orientation and staff development programs Supervising preparation of payroll data DIRECTOR OF NURSING, CLINICAL MANAGER OVERSIGHT The DON, Clinical Manager shall establish process to verify that skilled nursing and personal care services were provided. PROCEDURE: 1) The Director of Nursing, Clinical Manager shall establish a process to verify that skilled nursing and personal care services were provided. When requested by a State Regulatory Agency employee, the Director of Nursing, Clinical Manager shall provide a certified report that lists the home health services provided by a specified direct service staff person or contracted staff person for a specified time period. A certified report shall be in the form of a written or typed document or computer printout and signed by the Director of Nursing, Clinical Manager. The report must be provided to the surveyor within two hours of the request, unless the time period requested is longer than one year, then the report must be provided within three hours of the request. 2) The Director of Nursing, Clinical Manager shall establish, and conduct an ongoing quality assurance program. The program shall include at least quarterly, documentation of the review of the care and services of a sample of both active and closed clinical records by the Director of Nursing, Clinical Manager or his or her delegate. The Director of Nursing, Clinical Manager assumes overall responsibility for the quality assurance program. Home Health Agency Policies A-35
