MDT Home Health Care Agency, Inc. (e) If an individual serves as the DON, Clinical Manager of more than one licensed agency, Pursuant to s.400.462 (10), F.S., a designated alternate DON, Clinical Manager must be available during designated business hours, at each additional agency, which has the responsibility and authority for the clinical operation. Available during designated business hours means being readily available on the premises or by telecommunications. DIRECTOR OF NURSING, CLINICAL MANAGER OVERSIGHT The DON, Clinical Manager shall establish process to verify that skilled nursing and personal care services were provided. The director of nursing shall establish a process to verify that skilled nursing and personal care services were provided. When requested by an AHCA employee, the director of nursing shall provide a certified report that lists the home health services provided by a specified direct service staff person or contracted staff person for a specified time period. A certified report shall be in the form of a written or typed document or computer printout and signed by the director of nursing. The report must be provided to the surveyor within two hours of the request, unless the time period requested is longer than one year, then the report must be provided within three hours of the request. The director of nursing shall establish, and conduct an ongoing quality assurance program. The program shall include at least quarterly, documentation of the review of the care and services of a sample of both active and closed clinical records by the director of nursing or his or her delegate. The director of nursing assumes overall responsibility for the quality assurance program. PHYSICAL REQUIREMENTS: 1. Able to speak, read and write in English. 2. Able read assignments, follow directions, 3. Able to communicate and respond clearly on telephone and respond to patient’s spoken needs. 4. The ability to physically transfer, lift or assist patients whose average weight is 160 pounds with or without the aid of mechanical devices. (When performing visits) 5. Able to spend 80% of the work standing and/or moving about. (When performing visits) 6. Able to walk, climb stairs, stoop, twist, bend and squat to perform essential job functions. *when performing visits) This Job Classification will have a Potential risk for Occupational Exposure to Blood and other Potential Infections body fluids, protective equipment will be provided by our Agency to limit the exposure and will promote self protection practices in the delivery of the Home Health Care, to provide appropriate treatment to home health care workers in the event of exposure incident and to promote compliance with the universal precautions. MENTAL REQUIREMENTS: 1. Able to concentrate on detail with frequent interruptions. 2. Able to follow, complete and remember daily routines and requirements. 3. Able to comprehend and utilize professional education materials. 4. Able to cope with the mental and emotional stress of the position. PROFESSIONAL QUALIFICATIONS: 1. Must be a direct employee of the agency. 2. Graduate of an approved School of Nursing/College Degree. 3. Licensed in the State of Florida. Home Health Agency Job Descriptions I-12
