CLINICAL MANAGER JOB DESCRIPTION Staff Name/Title: __________________________________________________________ POSITION SUMMARY The Clinical Manager provide oversight of all patient care services and personnel. Oversight must include the following: is responsible to makes patient and personnel assignments, coordinates patient care, coordinates referrals, assures that patient needs are continually assessed and assured the development, implementation and updates individualized plan of care. Also, is a Professional Staff who is accountable for maintaining the quality of patient care in accordance with the agencies objectives and policies, through planning, coordination, implementing and evaluating the home health services provided. Lines of authority and reporting responsibilities: Report to the Director of Nursing (if the Clinical Manager is not the DON), and to the Administrator. RESPONSIBILITIES 1. Coordinates and schedules the daily work load of each district utilizing agency personnel to their fullest capacity. 2. Provides daily contact supervision of field nurses, other healthcare professionals, and home health aides to assure the quality of services and maintenance of standards. 3. Reviews and investigates the clinical content, diagnosis, medical history, medications, fee status and other procedures of the record of patients under their supervision. 4. Directs staff toward the appropriate use of community resources. 5. Appraises performance at probation and annually with the Director of Nursing (if the Clinical Manager is not the DON). Provides the teaching and counseling necessary to enhance the delivery of services and Professional development of nursing, professional and health aide staff. 6. Identifies and evaluates nursing needs of the patient and appropriateness of home care by review of the initial intake screening form; prior to assignment of direct care staff. 7. Represents and participates in community related functions as necessary. 8. Assists with the orientation of new professional staff to agency policies and procedures. 9. Sees patients in the home setting as needed. 10. Takes on-call as directed. 11.Participates in audits, interdisciplinary team conferences and represents agency in the community as requested. 12.Submits to the Director of Nursing (if the Clinical Manager is not the DON) yearly self- evaluations, goals and objectives. 13.Reviews each nurse’s and other staff schedule for recertifications, discharges, and home health aide supervisory visits. 14. Reviews weekly schedule done by nurse and other staff for changes or updates. 16.Reminds each nurse to check Medicare, or any other program eligibility information every month. Notifies Board of Directors, DON (if the Clinical Manager is not the DON), and QAPI staff of any changes in status immediately. 17. Fills out the patient’s re-hospitalization referral/admission form sheet daily and gives to intake department with recertification date written. Review OASIS assessments, sign-up package weekly. l8. Does monthly management summary reports. Gives to Director of Nursing (if the Clinical Manager is not the DON). 19. Ensure HIPAA guidelines and procedures are maintained.
