PSYCHIATRIC NURSE JOB DESCRIPTION Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager/Nurse Supervisor, Administrator. RESPONSIBILITIES 1. Act as case manager. 2. Interprets the underlying meaning in the patient’s actions. 3. Evaluates the need for the psychiatrists to change the plan of treatment. 4. Regularly re-evaluates the patient’s needs. 5. Establishes and periodically reviews plan of treatment. 6. Initiates appropriate preventative and rehabilitative nursing procedures. 7. Administers medication and treatments only as prescribed by the physician. Also observes, evaluates, and reports to the physician the patient's reaction to drugs or psychiatric treatments, or there are deviations from the plan of care. 8. Informs the physician and other health personnel of changes in the patient’s condition and needs. 9. Counsels the patient and family in meeting nursing and related needs. 10. Acts as case manager and coordinates the efforts of the health team in planning nursing care to meet the patient’s total needs. 11. Shall provide progress reports to the physician patient’s condition changes. 12. Responsible for supervisory visits to the patient’s home if receiving care from a licensed practical nurse or a home health aide at least every sixty (60) days. 13. Assesses patients and family for appropriateness of acceptance for Home Health Care Services. Conducts regular or OASIS assessments accurately, according to instructions in the OASIS. 14.Implements medical treatment order, referrals, follow-up provision a supervision of nursing. Participate in discharge planning, in-services, evaluating outcomes of care. 15. Document all activities and findings in the clinical record. 16. Responsible for clinical record for each patient receiving care. 17. May assist selected portions of patient’s care to the licensed practical nurse or the home health aide under her supervision. 18. Initiates emergency measures in life-threatening or unsafe conditions until the physician can be made available. 19. Maintains the confidentiality of the patient’s clinical record. 20. Prepares and submits clinical notes. 21. Observes, monitors and records the patient’s responses to care ant treatment. 22. Ensure HIPAA guidelines and procedures are maintained. This Job Classification will have a Potential risk for Occupational Exposure to Blood and other Potential Infections body fluids, protective equipment will be provided by our Agency to limit the exposure and will promote self protection practices in the delivery of the Home Health Care, to provide appropriate treatment to home health care workers in the event of exposure incident and to promote compliance with the universal precautions. QUALIFICATIONS 1.Shall be graduate of an approved school of nursing. 2. Currently licensed in the State of Florida 3. Must have special training or experience which would include a Master Degree in Psychiatric or Mental Health Nursing or equivalent training. 4.Experience as a member of the nursing staff in an active treatment unit of a psychiatric hospital or experience as a member of an outpatient Mental Health Clinic or Hospital. 5. Must have four (4) contact hours for completion of HIV and AIDS education.
