MDT Home Health Care Agency, Inc. Managers/Clinical Supervisors The Managers/Clinical Supervisors assume responsibility for the provision of patient care services, based on plans of care authorized by the attending physicians The Managers/Clinical Supervisors are responsible for the following functions: Providing direct supervision to patient care staff in assessing, planning, and delivering patient care Supervising clinical staff in caseload management Conducting regular team/case conferences and other activities to maintain communication between staff assigned to patient’s care Providing orientation and staff development for assigned professional staff Providing periodic performance evaluations and clinical competency evaluations Developing and monitoring systems for clinical record documentation Receiving all patient referrals and assigning to appropriate professional staff Acting as liaison with physicians, payers, referring parties, and other agencies as needed Assisting Manager of QI in performing quarterly chart review and utilization review Participating in selection of processes and outcomes for performance improvement (QAPI) study Supervision of all paraprofessional staff that have direct patient contact PROCEDURE: · Functions, responsibilities, performance expectations, and supervision received and exercised are clearly defined in job descriptions for each classification of employee. · Each employee signs the job description indicating he/she has reviewed the job description and understands that he/she will be evaluated on the stated performance criteria. · Job descriptions and related policies are reviewed periodically and revised as indicated. · Job descriptions for newly created positions or extensively revised positions may be reviewed and approved by the Clinical Manager/Director of Nursing and/or Administrator, Governing Body, as needed.
