NURSE FIELD SUPERVISOR JOB DESCRIPTION Position Summary: The nurse field supervisor (NFS) (Registered Nurse) is responsible for assessing patient and family needs in order to promote the best care, the Agency can give in recovery and rehabilitation. All patients are to be assessed by the field supervisor at least once a month. This assessment is comprised of three things: 1- A nursing note 2- Monthly customer service status/satisfaction 3- NFS checklist All three must be turned in to the supervisor coordinator. Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager/Nurse Supervisor, Administrator. The nurse field supervisor has at least two (2) years of nursing experience , one of which has been preferably community health care experience, the responsibilities are as follows: The NFS verify that the disciplines seeing the patient are following their calendars as they should. Any problems are to be reported verbally, the same day as the visit, and also on customer service monthly questionnaire. The NFS are available and accessible during all hours of service. The NFS will evaluate the care given the patient and determine if it is up to the standards of Agency’s employees. Also, the NFS will assess patient satisfaction and determine what action is to taken to change the situation. This is to be documented on the customer service monthly questionnaire. The NFS will verify that the patient’s medications are still the ones listed on the referral sheet. If there is a change in the medications it needs to be documented on the NFS checklist. The NFS is to assess the patient’s needs and recommend any additional services that can or should be provided. This may include any lab work, durable medical equipment, wound care supplies or any other service that will improve the patient’s quality of care. The NFS will advise the office of any problems found during the monthly visits to the patient’s home. Ensure HIPAA guidelines and procedures are maintained. The NFS will perform other related duties as assigned by the Administrator, Director of Nursing, Clinical Manager, Direct Supervisor. PHYSICAL REQUIREMENTS: 1. Able to speak, read and write in English. 2. Able read assignments, follow directions. 3. Able to communicate and respond clearly on telephone and respond to patient’s spoken needs. 4. The ability to physically transfer, lift or assist patients whose average weight is 160 pounds with or without the aid of mechanical devices. 5. Able to spend 80% of the work standing and/or moving about. 6. Able to walk, climb stairs, stoop, twist, bend and squat to perform essential job functions.
