MDT Home Health Care Agency, Inc. The Administrator and/or Director of Nursing, Clinical Manager has ultimate responsibility for performance of all employees in the agency. This position oversees and directs the performance of the agency’s senior management staff by providing the following services: Conducting meetings to share information, provide guidance, and to discuss program development, implementation, and evaluation Monitoring staff performance in meeting goals in routine review of personnel appraisals Reviewing management information system reports The Administrator oversees and directs the performance of the Director of Nursing, Clinical Manager and supervisors/managers through the following activities: Monitoring staff performance in meeting goals via individual update conferences Using telephone conferences for resolution of patient and staff problems Conducting management meetings to share information and provide guidance Reviewing monthly activity reports for each clinical area Reviewing supervisors’ annual written evaluation of staff with recommendations for improvement. Reviewing annual and new staff written performance evaluations with staff, as well as discussing recommendations for improvement. The Director of Nursing, Clinical Manager oversees and directs the performance of Managers/Supervisors through the following activities: Monitoring staff performance in meeting goals via individual update conferences Using telephone conferences for resolution of patient and staff problems Conducting management meetings to share information and provide guidance Reviewing monthly activity reports for each clinical area Reviewing supervisors’ annual written evaluation of staff with recommendations for improvement Reviewing annual and new staff written performance evaluations with staff, as well as discussing recommendations for improvements. Managers/Clinical Supervisors supervise the performance of direct service and clerical staff assigned to them by providing the following functions: Regularly planned individual conferences with staff. Retrospective and concurrent record review and case management as per established plan On-site supervision of home visits for field staff Review of annual and new staff written performance evaluations with staff and recommendations for improvement.
