MDT Home Health Care Agency, Inc. POLICY ON PATIENT VISITS It is the policy of MDT Home Health Care Agency, Inc. that patient visits shall be made according to the following criteria: - Registered Nurses/Registered Therapists shall make supervisory visits to Home Health Aides at least once every two weeks for patients with skilled services, and every 60 days, for Aide without skilled services, Personal Care, Homemaker/Companion Services, or according Patient request for Private Patients . The Agency shall obtain authorization from the client to perform such supervisory visit, (may be including in our Service Agreement). - Registered Nurses shall make supervisory visits to Licensed Practical Nurse (LPN) at least once every month(every 30 days). - Said supervisory visits shall be arranged in such a way that some visits shall be announced, while other shall be unannounced. - Routine visits by RNs, LPNs, therapists, and Home Health Aides shall be made at such frequencies as are stipulated in the patient's Plan of Treatment, which is jointly planned by this agency and the patient's physician. - Registered Physical Therapist/Registered Occupational Therapist shall make supervisory visits to Physical Therapist Assistant (PTA) or Occupational Therapist Assistant (COTA) at least once every 30 days.
