MDT Home Health Care Agency, Inc. PATIENT ASSESSMENT FUNCTIONS AND QUALIFICATIONS PURPOSE: · To provide appropriately qualified staff to meet the patient’s initial and changing needs for appropriate care, treatment and/or services identified during the referral, assessment and/or reassessment process. · To ensure screening, assessment and reassessment activities are within the scope of practice, state licensure laws, applicable regulations or certifications of the discipline performing the assessments. POLICY: · MDT Home Health Care Agency, Inc. provides care, treatment and/or services consistent with the respective scopes of practice of the members of the healthcare team under the direction of a licensed physician. Qualifications may vary based upon the national and state clinical board requirements. · MDT Home Health Care Agency, Inc. provides and modifies care, treatment and/or services to meet the patient’s needs as identified through assessments and reassessments. PROCEDURE: · MDT Home Health Care Agency, Inc. strives to meet patient needs for care, treatment and/or services by providing the following services under the direction of a physician. Skilled Nursing: · Evaluates the patient in his/her place of residence or hospital for appropriateness of the care, treatment and/or services requested. · Completes the admission assessment of all patients accepted for care, treatment and/or services by MDT Home Health Care Agency, Inc. in conformance with Agency policy and applicable laws and regulations. · Assesses the physical, cognitive, behavioral, emotional, psychosocial and rehabilitative needs of the patient, including management of pain when appropriate. This assessment identifies facilitating factors as well as potential barriers to the achievement of patient goals. · Monitors patient’s clinical status and coordinates care provision with other members of the healthcare team. · Evaluates the patient’s response to the plan of care in a timely manner as required by Agency policy and applicable laws and regulations. · Reassesses the patient to determine the ongoing needs for care, treatment and/or services and progression toward goals as stipulated by Agency policy. Qualifications: 1. A graduate of an accredited School of Nursing and be licensed in the state of Florida. 2. Complete knowledge of nursing principles and procedures of skills in the technique of good patient care. 3. Good mental health and the quality of retaining emotional stability in situations of varying circumstances. 4. Minimum of one year experience, preferable in community health. 5. Meets the physical and health requirements of the Home Health. Physical Therapy: · Evaluates and provides care, treatment and/or services for the patient with neuromusculoskeletal disorders.
