PROBLEM CONCLUSION PURPOSE: To present study/problem topic results in the context of the specific topic To formulate judgment or opinions based on the study/problem topic To explain formulated study/problem topic conclusions and the rationale for the same To answer the study/problem topic question problem or hypothetical POLICY: Conclusions are based on analyzed and interpreted study and problem topic data. PROCEDURE: Conclusions are derived from analyzed and interpreted data. Conclusions focus on: Presentation and interpretation of topic/problem results. Implications of study topic/problem results within broader context. Application of conclusions to study/problem topic question problem or hypothesis. Conclusions are documented on a Performance Improvement Activity Form. Conclusions are communicated to appropriate individuals, departments and committees as necessary. Patient confidentiality will be maintained per our Agency policy and procedure. 84
