c. The agency uses an acceptable method of determining cost and change. d. The agency presents a financial statement from the current or previous year. This statement includes sources of income, showing percentage of total income each source represents. 5. Statistics. The agency produces statistical data, including at least the following: a. Kind and amount of service provided, including all disciplines involved. b. Number and characteristics of patient under care (e.g. referral source, diagnosis, age,.etc.) c. Source of payment d. Therapies provided (If Applicable) e. Visits provided by each category of worker. 6. Administrative polices: The agency has states policies that insure adequate protection for patients and agency staffs, such as liability and other insurance, provisions for confidentiality and retention of records, personnel policies. 7. Program objectives and activities: a. The agency’s overall program objectives are clearly developed and priorities are defined. The major program is a family-centered services devoted to care of the sick. Objectives for such a program include: 1. Evaluating the health programs, marketing strategies, and needs of the ill at home. 2. Provision of preventive, curative, ameliorative, and rehabilitative home health services to assist patients and families to cope with their illnesses. 3. Encouraging maximum degrees of independence possible that appropriate goal, or permitting the degree of dependency necessary for the well - being of the patient or family. 4. Participating in the teaching of good health practices to avoid complications or to deter further deterioration. 5. Counseling patients and families in the management of their health problems or providing pertinent and appropriate information for the patient and his/her family to make decisions about their own health care. 6. Identifying, at the earliest possible stage, factors or situations which are real or potential and supervision to prevent or minimize their consequences. 7. Coordinating the various disciplines involved referrals from agencies. 8. Assuming and advocate role on behalf of patients and their families, as needed in relation to identified health problems. a. Basic services are provided to implement theses objectives and maintain patients in their own homes. 1. These basic services includes: a. Nursing services b. Therapy services c. Medical supplies 2. Services are provided directly by the agency or through contractual arrangements with another agency. b. Examples of activities to carry out this program areas are: 1. Provide care and carry out treatment ordered by a physician (such as injection, sterile dressing, or insertion of catheters, etc). 2. Provide physician directed rehabilitation and maintenance services for persons disabled by arthritis, stroke, fractures, etc. 3. Collaborate with physicians and other health professional to monitor patients health status and maintain them at an optimal level of functioning. 4. Teach patients and families good health practices and self-care. 5. Evaluate and assess health needs to assist patients and families to obtain and use needed health and social services available in the community. B. OPERATION OF AN AGENCY I- Implementation a. Staffing 1. The agency conducts or provides for orientation of new staff and a program of on-going in- service education. 2. The agency provides supervision for professional and nonprofessional workers whether on agency staff or purchased from another agency; these workers meet the criteria of the appropriate 8
