II I . QAPI Program Evaluation a. The agency has established procedures for evaluation of appropriateness, quantity, and quality of its services. 1. Utilization Review 2. Peer Review Audit 3. Case Record Review studies b. The agency pursues on-going evaluation of its staffing patterns and policies 1. Review of statistical data (e.g. admissions frequency of visits, length of stay) 2. Review of productivity reports (e.g. average visits per day), review of patient case-load per primary - case coordinator. 3. Review after modification in programs and practices. c. The agency has established methods and procedures for evaluation of staff performance. d. The agency has established procedures for evaluation of its programs and operations: 1. Review of statistical data desired from both the agency and the community. 2. Review by committees of Board of Directors (e.g. Clinical Department , Finance and Program Planning Committees) to assess program effectiveness. I V . Expansion of services a. The agency is aware of and attempts to be responsive to community needs. (Evidence of this might include formalized approaches to long-range planning (e.g. position statements, community studies, and review and restatement of agency purpose and objective). b. If the agency has had some experience with adding new services through special funding, such new services have been incorporated into the on-going program. c. Where other health organizations and institutions exist in the area, the agency has developed cooperative programs and policies that will contribute to smooth expansion of services. 10
