QUALITY IMPROVEMENT ASPECT OF CARE, INDICATOR, THRESHOLD Aspect Client/Patient Accident Indicator Clients receiving Agency services are maintained safely within service, place limitations whether: 1. Clients have been instructed on prevention of Accidents, including: • Fall Prevention • Fire Prevention • Safety at Home, including all areas (bedroom, bathrooms, stairs, etc) • Safety use of Medical Device, equipments, Oxygen 2. Clients are able to reach the Agency when they need to report any accident, when are admitted to the Agency, but none agency’s staff is in their home at time of accident 3. Patient’s Physician are informed of Accident, any follow up order are taking and compliance enforced. 4. Accident Log are up to date, with separated numbers for witnessed and unwitnessed. Threshold 100% of Accidents Sample Random a minimum of 90% of quarterly accidents Methodology Monthly client accident incident is investigated, solution plan to prevent further accident is established. Responsibility Results will be submitted to the Director of Nursing, Clinical Manager or his/her designee whose responsibility it shall be to channel any identified problems to the appropriate staff for resolution and follow-up. Re-evaluation: next quarter Plan if finding fail to meet acceptable standards: In-service to staff, Plan to Prevent further accidents is implemented. 123
