Home Health Care staff and interdisciplinary team members inform the Director of Nursing, Clinical Manager of Home Health Care in the event of any problems arise in the discharge planning or client discharge that the staff or team members are unable to handle. 5. Other criteria for discharge is governed by any of the following: a. Client recovers to a level no longer requiring skilled nursing or other services on a part-time, full-time or as intermittent basis. b. Client no longer homebound if required for reimbursement. c. Client or responsible persons refuse to cooperate with treatment provided by the Home Health Care Agency or adhere to physician’s orders. d. Services may be terminated by the physician at any time. 47
