Discharge to Community Needing Wound Care or Medication Assistance GOAL: Discharge to the community and self-sufficient with wound care and medication regimen Plan: 1. Assess ability to perform wound care and medication management within first week of services. 2. If unable to perform wound care and/or medication management, identify caregiver. 3. If unable to perform wound care and/or medication management, contact physician and obtain referral for social work services. 4.
