QUALITY IMPROVEMENT ASPECT OF CARE, INDICATOR, THRESHOLD Aspect Adverse Events from OASIS OBQI reports Indicator Incidence rates for infrequently occurring untoward Adverse Events (outcomes). Threshold 100% of Adverse Events from OBQI reports Sample Random a minimum of 90% of quarterly Adverse Events, Outcomes Methodology Monthly Adverse Events, Outcomes is investigated, solution plan to prevent further Adverse Events is established, in a quarterly basis, as part of Professional Advisory Committee duties. Determination of the motive of the Adverse Event: Medication error, equipment, intervention, underlying disease, route of administration, staff error, procedure deviations, Other suspected cause. Responsibility Results will be submitted to the Director of Nursing, Clinical Manager or his/her designee whose responsibility it shall be to channel any identified problems to the appropriate staff for resolution and follow-up.
