MDT Home Health Care Agency, Inc. Home Health Care staff member. 3. Arranges subsequent meetings as appropriate with the staff member to assess progress with the plan of corrective action. 4. Documents subsequent meetings with the staff members on the Inappropriate Behavior Documentation form. 5. Gives copies of completed Inappropriate Behavior Documentation forms on such meetings to the staff member. f. If the corrective action is not taken by the staff member within the defined time period, or if the staff member demonstrates further inappropriate behavior, the Director of Nursing, Clinical Manager of the Home Health Care: 1. Implements the next step of the disciplinary process: a. First step: formal counseling b. Second step: verbal warning c. Third step: written warning d. Fourth step: suspension e. Fifth step: termination 2. Documents the next step of the disciplinary process on the Inappropriate Behavior Documentation form. 3. Gives a copy of the Inappropriate Behavior Documentation form describing the next step of disciplinary process to the Home Health Care member. g.
