MDT Home Health Care Agency, Inc. DISCIPLINE OF HOME HEALTH CARE STAFF MEMBER POLICY Discipline of home heath care staff members for inappropriate behavior is imposed in a fair and consistent manner. PURPOSES 1. To assist individual home health care staff members in addressing inappropriate behavior. 2. To provide a fair and consistent approach in addressing inappropriate behavior by Home Health Care staff members. PROCEDURES 1. Definition: Inappropriate behavior: Home Health Care Staff Member actions that adversely affect the quality of care, personal safety, or comfort of clients or the proper functioning of the Home Health Care Agency’s operations, services, programs, activities and staff. 2. Action: a. Inappropriate behavior demonstrated by a Home Health Care staff member is documented on a Staff Concern form (See Staff Concern form). b. The completed Staff Concern form is submitted to the Director or Nursing of Home Health Care. c. The Director of Nursing, Clinical Manager of Home Health Care: 1. Investigates the Staff Concern report, as appropriate. 2. Reviews and discusses the Staff Concern report with the Administrator of Home Health Care. 3. Schedules a meeting with the staff member who reportedly demonstrated the inappropriate behavior. d. In the meeting with the staff member who reportedly demonstrated the inappropriate behavior, the Director of Nursing, Clinical Manager of Home Health Care: 1. Considers the following courses of action with regard to the staff member’s reported inappropriate behavior: a. Formal counseling b. A verbal warning c. A written warning d. A suspension notice e. A termination notice 2. Discusses the inappropriate behavior with the staff member. 3. Clarifies relevant issues or problems from a management perspective. 4.Notes desired corrective action to be taken within a defied time period. 5.Notes the possibility of further disciplinary action if such corrective action is not taken in the stipulated time period. 6.Notes that all documentation regarding the inappropriate behavior will be placed in the staff members personnel file. 7.Encourages the staff member to prepares a written explanation for the inappropriate behavior. e. The Director of Nursing, Clinical Manager of Home Health Care subsequently: 1. Documents the meeting with the Home Health Care staff member on an Inappropriate Behavior Documentation (See Inappropriate Behavior Documentation Form). 2. Gives copy of te completed Inappropriate Behavior Documentation form to the Home Health Agency. - - Personnel/Operations Policies B-58
