MDT Home Health Care Agency, Inc. DENTAL INSURANCE References: Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) Summary Plan Description POLICY: MDT Home Health Care Agency, Inc. may provides eligible direct/full time Employees and their dependents with dental insurance effective the first day of the month following 30 days of employment. (Final decision by the Board of directors, depend of Agency’s economical situation) PROCEDURE: · Regular full-time and permanent part-time Employees are eligible to participate in the dental plan and may enroll during the first week of employment. · Individual coverage is provided by MDT Home Health Care Agency, Inc. at no charge to the employee. The cost for family coverage will be provided in the new-hire packet and upon request from the Human Resource Department. · Employees who fail to enroll during the first week of employment must wait until open enrollment as established by the company. · Contact the Supervisor or designee or the Human Resource Department for enrollment applications. · An Employee who terminates employment or who changes status to PRN or is not consistently working at least 24 hours or more per week as a part-time Employee, is ineligible to participate in the dental insurance benefit plan. The Human Resource Department will issue a COBRA letter to notify the Employees of their right to continue coverage. · Coverage will terminate the last day of the month following the change in status or termination of employment. · A Plan description is provided in the Description of Benefits manual. Contact Human Resource to ensure the Plan Description is the most current. · Plan Descriptions are updated annually. Each year the new plan updates supercede the prior years plan. · MDT Home Health Care Agency, Inc. reserves the right to change or modify benefit programs without prior notification to the Employee except where required by law.
