MDT Home Health Care Agency, Inc. HEALTH REQUIREMENTS POLICY: Each field Employee is required to meet applicable health regulations, which may include a health review. When a health statement is required, it is to be completed after the field Employee has accepted the conditional job offer, prior to the first day of work and annually thereafter. Should the health review be unsatisfactory, the Human Resource Department must assist in determining if a reasonable accommodation would be appropriate as required under the Americans with Disabilities Act. PROCEDURE: The field Employee must complete the health questionnaire during the orientation process and annually thereafter. The field Employee who is unable to have a TB skin test must have a chest x-ray completed at the Health Department prior to making any patient visits. Evidence of TB test, chest x-ray within the last twelve (12) months or Medical certificate is enough to qualify for employment. · If a health review is required as part of the hiring process, the prospective field Employee should be informed that the offer is conditional upon the satisfactory result of a post offer health review. · The time taken for a health review prior to the field Employee’s first day of work is not work time. · If the medical review reveals a disability that prevents the applicant from performing the essential job functions of the position, the Director of Nursing, Clinical Manager, in conjunction with the Administrator must determine if a reasonable accommodation is available that would enable the applicant to perform the essential job functions, or if the accommodation would impose an undue hardship on operations. If reasonable accommodation is available for the applicant, the applicant should be hired if he/she is qualified for the job. If a disabled applicant cannot be reasonably accommodated or is not otherwise qualified for the job, he/she should not be hired (Refer to Policy “American with Disabilities Act” for details). · Field Employees are responsible for providing the Company with updated health changes if the change relates to the field Employee’s ability to perform the essential job functions. · The tuberculosis skin tests must be done, prior assignment any patient (for staff that do not have proof of TB/chest x-ray done in the past 12 months), and a one step skin test annually or Medical Certificate stating that the Employee is free of TB symptoms within 30 days to the field Employees’ annual employment anniversary date. . A repeat chest x-ray will only be required if a field employee develops symptoms that warrant a chest x-ray or if recommended by the field employee’s physician. · Health statements may be completed by the field Employee annually after hire ( within 30 days to the field Employees’ annual employment anniversary date) Required medical information must be kept in a health information envelope, separate from the regular personnel forms, inside the file in a separate envelope, labeled “Confidential”.
