POLICY ON DRUG ORDERS AND CHANGES IN ORDERS (That are subject to Federal and State Controlled Substance Acts) It is the policy of this agency that each original physician's drug order, as well as any subsequent change in order, shall be signed by the physician. For this purpose, our form on "Order for Drugs" shall be signed by the physician and shall be incorporated in the patient's records as maintained by our office. Further, it is our policy that any changes in orders for the administration of such drugs that are subject to Federal and State Controlled Substance Acts and other legend drugs, i.e. requiring prescriptions, shall be signed by the physician. Any verbal authorization by the physician to change drug orders shall be reviewed and signed by the same physician within thirty (30) days after order is issued. Only registered nurses are authorized to take verbal orders on medications for patients. All prescriptions must be made (written) by a physician and must indicate: a) Dosage; b) Name of the medication; c) Frequency of administration; and d) Route of administration.
