MDT Home Health Care Agency, Inc. ! Help developing and implementing the patient care plan for the patient, perform an evaluation/assessment including OASIS (if applicable). ! Providing nursing services, treatments, and preventive procedures as appropriate for the patient's care and safety. ! Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs, and changes in the patient's physical or emotional condition. ! Teaching, supervising and counseling the patient and family members regarding the nursing care needs and other related problems of the patient at home. ! Supervising and training other nursing service personnel. ! Coordinating total patient care. ! Nurse Clinician available for consultation with physician and to provide specialized care and education. ! Inclusive discharge summary for physician. All nursing services require a physician's order and are provided according to the specific directions of the patient's attending physician. PHYSICAL THERAPY Registered Physical Therapists will, on the order of a physician, evaluate patients and make up a plan of treatment to individual's needs. These services may include: ! Utilization of modalities (i.e., heat and ultrasound) in conjunction with an active exercise program, evaluate the need of devices. ! Gait training, strengthening exercises and functional training in activities of daily living to promote maximum independence. ! Teaching appropriate transfer techniques dictated by the patient's disability and home situation. ordering specialized equipment and modifying home environment to enhance safety and independence. ! Consulting with physician, development of Plan of Care, Evaluation/assessment of patient, including OASIS if applicable. ! Evaluation of outcome of care, supervise PTA and HHA as needed SPEECH THERAPY The certified Speech Pathologist will evaluate, plan and execute a program of treatment for communication disorders. These services include: ! Evaluating patient for expressive and receptive language disorders, conduct an OASIS assessment if applicable, evaluation of outcomes of care. ! Planning and providing a comprehensive speech therapy program for the patient and instructing the family in techniques and rationale, coordination of care with other discipline as applicable, participate in discharge planning. ! Contacting patient's physician regarding goals, treatment and progress, development the Plan of Care OCCUPATIONAL THERAPY Occupational Therapy is purposeful activity designed to enhance the physical and psychosocial functioning of the individual. This service may include: ! Evaluating and establishing goals on the patient's current level of functioning, evaluate outcome of care, participate in the Plan of care development ! Constructing adaptive devices and splints if needed, conduct assessments/evaluation as needed, Home Health Agency Bylaws H-4
