MDT Home Health Care Agency, Inc. HOME HEALTH SERVICES PROVIDED PRIVATE INSURANCE/MEDICARE/MEDICAID/RECIPIENTS Medicare/Medicaid pays the full cost of each home health care visit, provided that: * The attending physician, physician assistant, or advanced registered nurse practitioner, acting within his or her respective scope of practice orders the service. * The patient is essentially homebound due to illness or injury. * The patient may benefit from one or more of the following: Skilled Nursing Care, Physical Therapy, or the services of a Home Health Aide. ALL NON-MEDICARE/MEDICAID PATIENTS (e.g. GROUP INSURANCE POLICYHOLDERS) Patients' treatment is pursuant to treating physician's orders in conjunction with individual company's home health care policy provision. WHAT IS HOME HEALTH CARE? It is a series of services (skilled nursing, physical therapy, speech therapy, home health aide, social work, occupational therapy, and nutritional guidance; available to the patient in his home environment. Services are rendered on a part-time basis. WHY HOME HEALTH CARE? ! Permits patient to be in familiar surroundings. ! Releases needed hospital beds. ! COSTS less than hospital care. ! Teaches patient and/or family to deal with patient care in the home. WHEN HOME HEALTH CARE? When patient: ! Is basically confined to home and cannot function in community without assistance. ! Is under physician's care. ! Receive services under a plan of care established and reviewed by a physician ! Will benefit from skilled nursing, physical therapy, or home health aide services. ! Have had a face-to-face encounter with a physician or allowed Non-Physician Practitioner(NPP) (Medicare receipients) WHO MAY ORDERS HOME HEALTH CARE? The attending physician, physician assistant, or advanced registered nurse practitioner, acting within his or her respective scope of practice. OFFERS THE FOLLOWING COMPREHENSIVE RANGE OF SERVICES: ! Skilled nursing care ! Home health aide service ! Physical therapy Personnel meets all federal and state requirements relevant to the practice of their professions. "Skilled care" means nursing services or therapeutic services required by law to be delivered by a health care professional who is licensed under part I of chapter 464; part I, part III, or part V of chapter 468; or chapter 486 and who is employed by or under contract with a licensed home health agency or is referred by a licensed nurse registry. SKILLED NURSING CARE Professional Nurses will, under the Plan of Care established by the physician, render the skilled service in the patient's home. The nurse remains in the home for the amount of time needed to carry out the physician's orders. “Nursing care” means treatment of the patient’s illness or injury by a registered nurse or a licensed practical nurse that is ordered as required in 400.487(2), F.S. and included in the plan of care.
