MDT Home Health Care Agency, Inc. JOB DESCRIPTIONS POLICY: Home Health Care staff positions are defined by written job descriptions. Written job descriptions are reviewed at least annually and updated as needed. The Agency’s job descriptions are consistent with the Agency organizational chart with respect to function and reporting responsibilities. Receipt and/or review of the job description with personnel is part of the orientation process and is repeated during the annual performance evaluation and whenever the job description changes. We verifies the receipt and review by giving personnel a copy of the job description. Our Agency requires personnel to sign a copy of the job description and places the signed copy in the personnel file. PURPOSES 1.To define standards for Home Health Care staff role performance. 2.To document Home Health Care members job responsibilities and expectations. PROCEDURES 1.The written job descriptions includes: a. Job title: name by which position is identified. b. Scope of position: overall statement of responsibilities and duties. c. Qualifications: necessary education or competencies, years of experience and other necessary requirements. d. Physical requirements: expectations of what is physically required to fulfilled the responsibility of the position. e. Performance responsibilities: Job performance expectations. f. Organization relationship: the positions inter and intra departmental reporting cooperative relationship. g. Acceptance of position: I have read and understood the job description for position and to meet the define standards. h. Signatures: the signatures of the Home Health Care staff members and the date of their signature. 2. Job descriptions are documented for each Home Health Care Agency, position for example, Administrator of Home Health Care, Director of Nursing, Clinical Manager of Home Health Care, Registered Nurse, Licensed Practical Nurse, Home Health Aide, etc. 3.
