MDT Home Health Care Agency, Inc. JOB DESCRIPTION CHORE WORKER The Chore Worker is matched with an Elderly, Disabled or Chronically Mentally ill adult to help with grocery shopping, medical appointments or other errands as needed and requested by the social worker. This may be a one time assignment or on going. Errands may be done with the client or on own, depending on the client’s abilities/wishes. Responsibilities: 1. To transport passengers in a safe and lawful manner. This includes the use of seat belts for all passengers. 2. To be prompt in keeping appointments and adhering to agreed upon schedules. Keeping in mind that schedules do fall behind due to unforeseen events. 3. To abide by the Agency’ Rules of Confidentiality and HIPAA guidelines. 4. To record time and mileage using the assigned code number and forms. To turn in these forms in a timely manner so that reimbursement can be made and the files kept up to date. 5. To report as soon as possible any problems or concerns regarding a specific client or transportation request. 6. To work with the Agency’s Coordinator, Social Worker and any other collaborating agencies as a team member to best meet the needs of the client. 7. To inform the Agency’s Coordinator of vacations, illness etc. that would necessitate a substitute driver, with as much advance notice as possible. 8. To treat clients with respect and dignity. 9. To perform only those chore requests authorized by the Agency’s Coordinator, at the request of a Director of Nursing, Clinical Manager, to fulfill the case plan for that individual/family. Assignments May Include: · Shopping for groceries. Heavy and simple household chores · Shopping for personal items · Transporting to medical appointments, Transporting to other appointments authorized by the Agency’s coordinator . Seasonal maintenance of the participant’s propriety Qualifications: · Should be able to lift grocery bags · Should be sensitive to the needs of the elderly/disabled. Complete at least 4 hours of training. · Should be dependable, flexible and patient · Must hold a valid driver's license. · Must carry the state legal limits of liability insurance on their vehicle. The Agency recommends a higher limit of 100-300-100. · Must provide his or her own vehicle to perform this duties. Time Commitments: The chore worker decides how busy they want to be during the week/month. Each assignment may be accepted or denied according to the worker’s schedule. He/she can also set up their own schedule to coincide with the clients on most occasions. Assignment Procedure: All requests for chore services must have been authorized by the Agency’s coordinator and referred to assignment. Assignments may be for one time or they may of an ongoing nature. The worker accept or decline assignments from the Coordinator. If the Agency ask for changes or new requests, they should be referred back to the Coordinator.
