MDT Home Health Care Agency, Inc. • Makes initial and periodic evaluation of patient’s respiratory therapy needs, including outcomes of care, and discharge planning. • Communicate in writing and/or verbally with the referring physician, physician assistant, or advanced registered nurse practitioner, acting within their scope of practice and / or other Home Health Care Agency personnel to obtain and give pertinent information on the patient’s progress and/or plan of care desviation. • Record content of each visit made to the patient. Provides information for development of re-certifications. Instruct the patient and caregiver in care and use of respiratory therapy devices; instruct/consult other health team personnel including, when appropriate, home health aides and caregivers in certain phases of respiratory therapy in which they may assist the patient; and instruct the patient and caregiver on the patient's total respiratory therapy program. Monitoring respiratory equipment for safety and appropriateness. Qualifications: 1. Is a graduate of a Respiratory Therapy school approved by the council of Medical Education of the AMA in collaboration with the National Boards of Respiratory Therapist or its equivalent. 2. Is licensed to practice in the State of Florida. 3. Has a minimum of one (1) year experience as a Respiratory Therapist. 4. Meets the health and physical requirements of the Home Health Care Agency. Medical Social Services: · Evaluates the social, emotional, economic and environmental needs of the patient and his/her family in his/her place of residence. · Evaluates the patient’s/family’s response to the social services plan of care in a timely manner as required by Agency policy and applicable laws and regulations and coordinates service provision with other members of the healthcare team and appropriate community resources. n Reassesses the patient and his/her family to determine the ongoing needs for social services and progression toward goals as stipulated by Agency policy. Qualifications: Master's degree in Social Work or other social sciences from an approved university, accredited school of social work. Must have at least one year's experience in social services. A Registered Nurse completes all initial evaluation assessments utilizing the Outcomes and Assessment Information Set (OASIS) except in those instances where the physician has ordered only therapy services. The Registered Nurse regularly reevaluates the patient’s nursing needs utilizing OASIS data items. If the physician orders only therapy services, it is acceptable for the appropriate therapist (Physical Therapist or Speech-Language Pathologist) to complete the initial assessment evaluation utilizing the OASIS, or the therapist may complete a therapy specific assessment form with the nurse completing the OASIS data set within five (5) days of the start of care date. Physical Therapists, Respiratory Therapists, Occupational Therapists, Speech-Language Pathologists and other disciplines assigned to the patient should assess and reassess the L-40 Home Health Agency Client Assessments
