Therapy Evaluation and Care Plan developed by our registered therapists shall be transferred to a Patient’s Plan of Care (485 Form) or Initial/Modify Order that is signed by Patient’s Physician within 30 days, for that reason the Physician Signature on the Therapy Evaluation or Care Plan Form is optional, and not required by our Agency. Experienced Pediatric Nurses should assess, evaluate and provide care to pediatric patients. Post childbirth home health visits should be conducted by Registered Nurses trained in maternal/child care. Certified/experienced psychiatric nurses should assess, evaluate and initiate nursing interventions for psychiatric patients.
