SOCIAL SERVICES Facilitate long term planning to promote independence and provide opportunities to take advantage of a variety of community resources designed to assist the elderly with their personal concerns and help alleviate their troubles. Community resource planning Identify eligibility for services/ benefits Nursing home placement assistance Alternate living arrangements Arrange transportation for medical appointments Emotional support to patient/client/family Financial resource information Arrangement of meal services Crisis intervention Referral to support group/community resource(s) HOME HEALTH AIDE SERVICES: Home Health Aides provide personal care to our patients under the supervision of a Registered Nurse or a Licensed Therapist. Personal care Assist with Dressing Assist with bathing Prepare, serve meals Light housekeeping Assist with ADL’s Pick up groceries and medications for patients at local grocery stores or pharmacy Assist with ambulation, transfers Hair Care Assist with self administration of medications Grocery Shopping Skin/Foot/Denture/Oral/Nail Care Waiver services: Homemaker, Personal Care, Respite, Companion, Sitter Assist with Ambulation/ROM/Mobility Assist with ambulation devices W/C / Walker / Cane Meal Preparation, Assist with feeding Wash Clothes, Light housekeeping Equipment Care, Assist with self administration of meds Exceptions, any special criteria for the acceptance of patients for service • The first dose of any IV medication must be administered under the supervision of a physician. • Where possible the first dose should be administered in the hospital prior to discharge to home health services. • First dose of investigational drugs must be given under the direct supervision of the physician. • Patients receiving drugs that have a high potential for causing anaphylaxis must not be given the first dose in the home, i.e., amphotericin B, chemotherapy agents. • Adherence to all other existing candidate selection criteria for home antimicrobial therapy is essential and must include knowledge of patient's complete medical, drug and allergy history, as well as a family allergy history. • If the patient has a history of allergy to the prescription drug, it must be changed to an appropriate alternate to minimize risk of anaphylaxis. • Prior exposure may have sensitized the individual; subsequent doses can elicit an allergic response. Special education, experience or licensure/certification requirements necessary for the clinical personnel to provide any special procedures or treatments. (Qualifications may vary based upon the national and state clinical board requirements) The Registered Nurse assigned to administer IV medications shall be qualified by education and experience and shall have been assessed as being competent by the Agency.
