MDT Home Health Care Agency, Inc. CONTINGENCY PLAN BACK-UP STAFFING A. In the event that a large number of clinical personnel were to terminate employment take a leave of absence, be out due to sickness or emergency situations, this home health agency shall schedule and utilize contract personnel to fulfill the Plans of Care established for the active patients on services. B. This Home Health Agency shall inform the active patients who will be affected, or the situation and of the new/temporary personnel who will be providing the care. C. This Home Health Agency may also contact a medical temporary placement service to service to contract experienced RN's, LPN's, home health aides, and physical therapists. Each discipline shall receive from this Home Health Agency. a. Orientation to the Agency policies and procedures. b. A copy of the Plan of Care for patients assigned. c. A list and daily/weekly visit schedule. d. Confidentiality of patient information form. D. The client may have a designee provide services. E. If the clients designee has agreed to provide back-up services, the agency will have the designee sign a written agreement to be the backup services provider. F.
