MDT Home Health Care Agency, Inc. PERSONAL LEAVE OF ABSENCE POLICY: MDT Home Health Care Agency, Inc. provides leave of absence without pay to regular full-time Employees and Permanent part-time Employees who wish to take time off from work duties to fulfill personal obligations. PROCEDURE: A. Regular full-time Employees and permanent part-time Employeesmay request personal leave only after having completed ninety (90) days of service. B. As soon as an eligible Employee becomes aware of the need for a personal leave of absence, he/she should request a leave from their Supervisor. C. Temporary leave may be granted for a period of up to sixty (60) calendar days every year. D. An Employee shall take any available PTO benefits as part of the approved period of leave. E. Requests for personal leave will be evaluated based on a number of factors, including anticipated work load requirements and staffing considerations during the proposed period of absence. F. The decision to grant or deny personal leave is in the sole discretion of MDT Home Health Care Agency, Inc. G. Subject to the terms, conditions, and limitations of the applicable plans, health insurance benefits will be provided by MDT Home Health Care Agency, Inc. until the end of the month in which the approved personal leave begins. H. When an Employee returns from personal leave, benefits will again be provided by MDT Home Health Care Agency, Inc. according to the applicable plans. I. An Employee may not work another job while on a personal leave of absence. J. The Employee must pay in advance the full cost of the health and dental insurance premium while on leave. MDT Home Health Care Agency, Inc. cannot be responsible for restoring benefits which have lapsed due to nonpayment by the Employee while on leave. K. PTO benefit accruals will be suspended during any leave greater than one month and will resume upon return to active employment. Any extended LOA may cause a change in the benefit date and annual increase. L. Employees must utilize PTO while on leave: · Employees receiving MDT Home Health Care Agency, Inc.-sponsored disability income insurance must utilize available PTO for the waiting period and a portion of each week. The total combined benefits paid shall not exceed the average weekly wage. · Employees not receiving MDT Home Health Care Agency, Inc.-sponsored disability income insurance must exhaust available PTO.
