MDT Home Health Care Agency, Inc. ATTENDANCE POLICY: Being on time and on the job is an essential part of an employee’s work performance. An employee is expected to notify his or her supervisor as soon as possible for any unplanned absence so arrangements can be made to fulfill the employee’s responsibilities for that workday. Procedure: · If an employee must report off duty, he or she should notify his or her supervisor or Manager on call as soon as possible. · Excessive absenteeism may result in disciplinary action including termination. · Excessive absenteeism is defined as greater than three (3) unplanned episodes of absences in a three (3) month period of time. · An episode is defined as any consecutive days of absence from work regardless of the number of days in the episode. For example, a call off for Monday, Tuesday then a completed workday on Wednesday with another call off on Thursday will count as two (2) episodes. Monday and Tuesday are consecutive days and count as one episode, then Thursday counts as a second episode. · Planned absences and requested days off, approved in advance, will not count as an “unplanned episode of absence” and will not result in disciplinary action.
