MDT Home Health Care Agency, Inc. AUTOMOBILE USAGE Policy: If applicable, the use of a company vehicle is an Employee benefit given to individuals. This benefit is a privilege, not a right, and the use of a vehicle can be revoked at any time. MDT Home Health Care Agency, Inc. cannot list rules to cover every situation, and Employees are expected to use good judgment with respect to the use of automobiles. PROCEDURE: Driving Records: MDT Home Health Care Agency, Inc. encourages Employees to drive safely on company time as well as personal time. Personnel required to drive for the company should: • Abide by all traffic laws. • Heed all traffic signs and signals. • Practice defensive driving. • Maintain good driving record. • Maintain current drivers license. MDT Home Health Care Agency, Inc.’s liability insurance may not permit an Employee to drive a company vehicle if they have a poor driving record. Each employee must have have a DVR before he/she will be allowed to operate a company vehicle. MDT Home Health Care Agency, Inc. considers an acceptable driving record to be no more than two (2) violations within the last three (3) years. Violations are not differentiated between work and non- working hours or whether the violation occurred prior to employment with MDT Home Health Care Agency, Inc. Employees charged with a DUI may not drive a company vehicle for a period of five (5) years. Employees are responsible for any fines or other penalties assessed against them. Accidents: The personal health and safety of every Employee is of the greatest importance to MDT Home Health Care Agency, Inc. and every attempt should bemade to reduce the possibility of accident occurrence. Seat Belts: It is a state law and MDT Home Health Care Agency, Inc. policy that seat belts be worn at all times. Security: Unattended vehicles must be locked at all times to prevent theft of company and/or personal property. Theft of personal equipment and other personal items left in your vehicle after working hours are the responsibility of the Employee and his or her personal insurance company. Reporting Accidents and injuries: All work related accidents and injuries, no matter how minor, should be reported to the Supervisor immediately. It is the policy of MDT Home Health Care Agency, Inc. to require that all Employees submit to a drug and alcohol test if involved in an automobile accident involving a company vehicle. MDTHome Health Care Agency, Inc. provides workers’ compensation insurance for every Employee. It protects the Employee for loss of pay and time and for the cost of medical care for injuries sustained while working. There is a three (3) day waiting period before compensation begins. If an Employee is involved in an accident, MDT Home Health Care Agency, Inc. requires: All information is obtained relating to the accident in a professional manner, including the names and addresses of any witnesses. The Employee may not negotiate the settlement of any claim, promise payment for any injury or Home Health Agency. - - Personnel/Operations Policies B-143
