MDT Home Health Care Agency, Inc. damage or admit liability. A written report for all accidents and injuries must be completed as soon as possible and include: A. A completed accident report provides necessary information to the Accounting Department to allow the processing of claims with insurance companies. B. Any damage to the company vehicle, no matter how slight, must be noted on the accident report. While driving a vehicle on company time, it is required that the Employees conduct be courteous and professional at all times and should not conflict with the ethical standards of MDT Home Health Care Agency, Inc. Smoking: Smoking is not permitted in company vehicles. Unauthorized Drivers: Only Employees of MDT Home Health Care Agency, Inc. are authorized to drive a company vehicle. Unauthorized/non-Employee use of a company owned vehicle is strictly prohibited. Personal Use of a Company Vehicle: If an Employee is involved in an accident on personal time, he or she will be responsible for the insurance deductibles. Maintenance: Vehicles must be maintained in accordance with manufactures guidelines. The following guidelines are to be adhered to by Employees who drive company vehicles: · It is the responsibility of the driver to make sure that the vehicle is equipped with proof of insurance, and current registration. These items are issued to the vehicle, not the driver, and must not be removed under any circumstances. (Auto ID cards are furnished by the Insurance Agent and must be in the vehicle at all times per State law). · To avoid unexpected down time and more costly repairs, it is required that all fluids (oil, water, transmission and brake fluid, etc.), belts and tires are serviced and checked per manufacturers guidelines. · The inside and outside of the vehicle must be kept clean. Use of Personal Vehicle: Individuals not assigned a company vehicle, may at times use their personal vehicle for company purposes. Reimbursement will be at the current company rate per mile or at the agreed upon travel stipend. This reimbursement is intended to compensate the Employee for all costs related to the operation of their personal vehicle on company business. The company assumes no liability for Employee vehicles used for work-related travel. Employees should not transport patients in their personal vehicle.
