PERFORMANCE NOTES POLICY: MDT Home Health Care Agency, Inc. is committed to high standards of work ethics and believes that Employees should be aware of the job responsibilities and expectations, as well as, the policies in which to adhered. Prior to the implementation of the Employee Performance Improvement Plan, a Supervisor should counsel (verbal or written) with the Employee in an attempt to correct the Employee’s deficient behavior. In addition, Performance Notes may also be utilized when commenting positively on an employee’s performance. PROCEDURE: · Performance Notes may be used whenever a Supervisor wishes to informally counsel with an Employee in writing prior to formal Improvement Plan. · Performance Notes should not be considered a formal step in the written Performance Improvement Plan. · Some offenses may result in immediate termination (see Personal Conduct Policy). · A Performance Note may be utilized as a summary of a verbal counseling or a summary of a written counseling. When the note is a written summary of a verbal counseling, an employee signature is NOT REQUIRED and a copy is not given to the employee. However, when the Performance Note is utilized as a summary of a written counseling, an employee signature IS REQUIRED and a copy should be offered to the employee. · If an Employee refuses to sign the report, the Supervisor will have a witness to the refusal while the Employee is still present. The Employee will be offered a copy of the Performance Note. · All Performance Notes should be filed in the employee’s personnel record. · Three (3) Performance Notes, verbal or written, documenting counseling for the same offense requires the employee be placed on a Performance Improvement Plan. The Performance Note may also be utilized as a summary of positive behavior by the employee. This summary does NOT REQUIRE employee signature but the supervisor is encouraged to share this summary note with the employee prior to placing original in the employee’s personnel file.
