MDT Home Health Care Agency, Inc. EMPLOYEE PERFORMANCE IMPROVEMENT PLAN POLICY: MDT Home Health Care Agency, Inc. is committed to providing quality care to the patients we serve. An employee’s quality of work is essential to meeting or exceeding these expectations. If an Employee’s quality of work consistently falls below expectations, the Employee may be placed on a formal Improvement Plan. PROCEDURE: · If performance issues arise, the Employee may be placed on a formal Performance Improvement Plan for a period of thirty (30) to sixty (60) days with a possible extension to ninety (90) days. · The Improvement Plan should contain specific, measurable goals for the Employee to achieve. All Employee Performance Improvement Plans should be approved in advance by the Director of Nursing, Clinical Manager. · The Improvement Plan should specify the next step in the process if improvement is not achieved. · A meeting should be held with the Employee routinely to discuss his/her progress or lack of progress on the Improvement Plan. · Minutes should be taken at each meeting by the Supervisor or designee. Each goal listed should be reviewed and discussed. · Both the Employee and Supervisor should sign and date the employee conference. The original minutes shall become part of the Employee’s personnel file. A copy should be given to the Employee. · An Employee whose performance does not improvemay receive additional disciplinary action up to and including termination. · An Employee on a Performance Improvement Plan is not eligible to transfer to another location. However, transfer to another position may be appropriate if the supervisor believes the employee’s performance would be improved in a different position.
