MDT Home Health Care Agency, Inc. POLICY ON GOALS OF A PLAN OF TREATMENT/CARE POLICY: It is the policy of MDT Home Health Care Agency, Inc. that each patient's Plan of Treatment/Care shall have specific goals, with endpoint parameters. Some goals will be short term, while others will be long term goals. While each case shall be planned to suit the patient, in general the goals of all treatment plans shall be the ultimate restoration of normal body functions to the patient. The goal is the complete recovery of the patient from his/her illness and the subsequent rehabilitation of the patient to include the resumption of the useful life of the patient. All goals shall be recorded on our standard Patient Plan of Treatment/Care form and must include the rehabilitation to potential and discharge planning. The frequency/duration of RN/RPT supervisory visits to each patient shall be once every fourteen days, 1 time a month, or every sixty days, according the Service and Program provided.
