Policy Manual sample

MDT Home Health Care Agency, Inc. 1. Biomedical waste, except sharps, shall be package in impermeable red, polyetheline or polypropyline plastic bag. Each plastic bag shall be constructed of polychlorinated free fill plastic and meet or exceed the following physical properties: (a) Impact resistance of 165 grams and tearing resistance of 450 grams in both the parallel and perpendicular planes with respect to the length of the bag. Impact resistance shall be determine using ASTMD-1709-91, and tearing resistance shall be determine using ASTMD-l921-89. 2. The biomedical waste generator shall have on file a bag quality test report supply by the bag manufacturer and performed by an independent tenting laboratory. A test report shall be prepared for each bag size used if the film is different between sizes or a change is made in the manufacturing process. The bag quality test report shall include all the following: a. Testing date; b. Bag Manufacturer; c. Bag dimensions; meaning length and width; d. Film gauge, meaning thickness; e. Impact resistance values, ASTM D-1709-91, f. Tearing resistance values, both parallel and perpendicular to the length of the bag, ASTM D-1922-99 and g. Name and address of the company that performed the test. 3. A written statement that the dyes used in coloring the bags meet the concentration levels for incidental heavy metals listed in 64E-16.004(2)(b)1.b., F.A.C., shall be included as a supplemental to the bag quality test report. 4. SHARP CONTAINERS: Sharp containers shall be segregated from all other waste and discarded directly into single use or reusable sharp container meeting the following minimum standard: a. Sharp container shalt be design for the containment of sharps. Milk jugs, coffee cans, or other type of container not design for the containment of sharps are not approved for use by generator; required to comply with subsection 381.0098(4) (a),F.S. b. Sharps container shall be clearly labeled with the phrase and International Biological Hazard Symbol. c. Dyes use in the construction or sharps containers shall meet the requirement for incidental metals. d. Sharps containers shall be leak- resistant, rigid, and puncture-resistant. e. Reusable storage container shall be rigid, leak-resistant and puncture-resistant under normal condition of handling and use and be constructed of smooth, easily cleanable, impermeable materials and be resistant to corrosion by disinfectant chemicals. Reusable storage containers shall be emptied directly into the treatment unit and shall be cleaned and disinfected immediately after each use. V. OFFSITE TRANSFER REQUIREMENTS. 1. Packages of biomedical waste shall remain intact until treatment. Ruptured or leaking packages of biomedical waste shall be repackaged prior to offsite transport. There shall be no recycling efforts nor intentional removal of waste from its packaging prior to the waste being treated. 2. Packages of biomedical waste shall be handle and transferred in a manner that does not impair the integrity of the packaging. Home Health Agency - - Skilled Professional Services
