MDT Home Health Care Agency, Inc. professionals involved in patient’s care. Complies with all applicable Policies and Procedures, Federal and State rules, regulations, and laws in effect, with respect to home health care. Promotes and participates in education activities for patient, Patient’s family, caregivers, and/or significant others (if necessary). Participates in personal growth and development. Documents all communications in the Communication Notes from the patients’ charts. The patient/client representative will be a qualified person who understands the principles of care for home health services, has the education necessary according to the requirements of his/her job description, and applicable law and regulation, has knowledge and experience, and has the orientation to all the care and/or services lie or she will he coordinating, understands the staff responsibilities associated with each level of care and services provided, has knowledge of the patient population served, has knowledge, and experienced appropriate to defined responsibilities and according to the Agency’s policy, has access to qualified consultation for services outside his/her expertise (if necessary). LINES OF AUTHORITY: The Administrator appoints the patient/client representative. The authority conferred by the Administrator is commensurate with his/her responsibility for the overall operation 0 f the Agency demonstrated by the job description, and by the organizational charts. The patient/client representative appointed by the Administrator has the necessary authority to implement his/her responsibilities for the operation of the Agency’s Patient Service Department. EVALUATION OF PERFORMANCE The performance of the patient/client representative shall be monitored regularly through a procedure established by the Administrator. The mechanism for evaluation, and the evaluation itself will be documented, and it may include, but will not be limited to achievement of objectives, performance appraisal, etc. REPORTING PATH: The patient/client representative will report to directly to the Administrator or to the office manager if such position exists. The patient/client representative may have supervisory responsibilities as indicated by the Administrator, when it is necessary, for the other clerical office staff, when the position of office manager does not exists.
